I’m a Doctor. Here’s what I tell patients over 50 who say they’re “not as sharp as they used to be.”

The name took a second longer to come back.

You knew it. You’ve known this person for years. And for one long beat in the middle of a conversation, it wasn’t there.

I’ve had this conversation in clinic more times than I can count. A capable person - running a company, a department, a household, sometimes all three - sits down and says some version of: “I’m not as sharp as I was. Should I be worried?”

The honest answer, for the overwhelming majority, is no. What they’re describing isn’t a disease. It’s a change in capacity - in how much mental energy is available for the demanding part of the day, and how fast it comes back after you’ve spent it.

And capacity is something you can work on.

What's Actually Changing

Your brain is roughly 2% of your body weight and uses something like a fifth of your energy. It is the most energy-hungry organ you own, and “being sharp” is, at the cellular level, mostly a question of whether your brain cells have the fuel to do demanding work on demand.

Deep inside every one of those cells is a molecule called NAD+. It’s central to how cells turn what you eat into usable energy, and to the day-to-day maintenance work that keeps cells running well.

Research suggests NAD+ levels tend to decline as we get older. Exactly how much varies by tissue and by study, so I won’t quote you a dramatic percentage - I distrust those when I see them in supplement ads. What I Tell Patients To Do First

I’m going to give you the same advice I give in the consulting room, in the same order, because if I skipped it you’d be right not to trust the rest of this page.

1. Sleep - actual sleep. Seven hours, most nights, without a screen in the last thirty minutes. Nothing on this page works as well as this does.

2. Move most days. Not a programme. A 30-minute walk. Strength work twice a week if you can. Movement is one of the most reliable things we know of for supporting the brain.

3. Look honestly at alcohol. Not “give it up” - look at it. For many people over 50 the two glasses that used to be fine are now costing them the next morning’s clarity.
