Not Every NAD Patch Works the Same Way

The word “patch” covers several very different delivery systems.

Standard adhesive patches

A standard patch places ingredients against the surface of the skin. The formula must pass through the outer skin barrier using passive diffusion or included penetration-support ingredients.

The presence of an ingredient inside the adhesive does not prove that a meaningful amount enters circulation.

Iontophoresis patches

Iontophoresis uses a mild electrical current to help move charged compounds through the skin.

Microneedle patches

Microneedle patches contain many small projections designed to cross the outer skin barrier.

Researchers have tested an NMN microneedle patch on pig skin outside a living body. The study showed that the experimental system could release NMN through the skin sample, but it did not test healthy-aging outcomes in people. (5)

That distinction matters.

An interesting laboratory result does not prove that every consumer NAD patch provides the same delivery.

The Skin Is a Barrier, Not a Sponge

Skin has an important job. It keeps water inside the body and many outside substances out.

One widely cited transdermal principle suggests that compounds need a molecular weight below roughly 500 Daltons for passive movement through intact skin. Larger molecules face a much harder journey. (1)

NAD+ has a molecular weight of about 663.4 g/mol, placing it above that general threshold. (2)

Molecular weight is not the only factor. Charge, water solubility, lipid solubility, formula design, skin condition, and delivery technology also matter.

What proof shows that this exact product crosses intact human skin in a meaningful amount?

A company should be able to answer that without hiding behind vague words such as “advanced” or “transdermal.”

What Human Research Exists on NAD Patches?

The research is smaller and less direct than the marketing category may suggest.

An eight-person institutional report

A 2019 annual report from the University of New South Wales described an experiment in eight adults aged 70 to 80.

Participants wore a 400 mg transdermal NAD+ patch for 24 hours. The report stated that plasma NAD-related markers changed after use. (3)

That sounds encouraging, but the evidence has major limits:

  • Only eight people participated

  • The group was aged 70 to 80

  • The report did not include a placebo comparison

  • It appeared in an institutional annual report rather than a detailed peer-reviewed paper

  • It cannot tell us how common consumer patches perform

This early observation may justify more research. It cannot settle the question.

A 36-person iontophoresis study

A 2024 pilot followed 36 people with persistent fatigue after COVID-19.

They used a 400 mg NAD+ iontophoresis patch once per week for 12 weeks alongside another active intervention. Participants reported changes in fatigue and quality of life, but the study had no placebo group and could not separate the patch’s contribution from the second intervention. (4)

The patch also used an electrical delivery system.

Those results should not be applied to a basic adhesive patch containing NAD+, NMN, or NR.

Experimental NMN microneedles

A 2023 study developed dissolving microneedles containing NMN and tested them on pig ear skin outside a living body.

The work showed that specially designed microneedles could cross the skin sample and release NMN. It did not measure blood NAD+ or personal wellness outcomes in humans. (5)

The practical takeaway is clear:

There is early research on advanced patch technologies, but evidence for common over-the-counter NAD patches remains limited.

Does This Mean NAD Patches Cannot Work?

No. It means shoppers should avoid grouping every product together.

A passive patch, an electrically assisted patch, and a microneedle system may contain similar words on the package while using very different delivery methods.

Before purchasing an NAD patch, ask:

  • Does it contain NAD+, NMN, or NR?

  • How much does each patch hold?

  • Is the listed amount the loaded amount or the measured delivered amount?

  • Does it use passive diffusion, iontophoresis, or microneedles?

  • Has this exact system been tested in people?

  • Were NAD+ levels measured?

  • How many people participated?
